I've been thinking a lot lately about the missteps in my DM approach. I was so focused on needing to have a complete story to be investigated I didn't give the appropriate attention to making it something that THESE CHARACTERS would want to investigate.
Part of that is something that could have been addressed up front, tying character creation to story creation in a more thoughtful way.
But then I kept doing it; NPCs killing each other off-screen, no one knowing why.
