S.T.O.P.
S - Stop what you are doing. Get to a safe spot to stop.
T - Take slow breaths. Slow in/out to the count of 5 sec. Also hold for 5 sec after each breath in and out.
O - Observe your surroundings. Take your mind away from what you are thinking and concentrate on colours, shapes, etc.
P - Proceed with caution. When you feel like you are better, proceed. Try not to fall back into what triggered you.
