Small decisions can be documented in code comments, but larger decisions (which often span multiple files or repositories) should be explained in more detail in Architectural Decision Records (ADRs, https://adr.github.io) or similar documents, separate from the code itself. Without a documented "raison d'être", no-longer-necessary code can hang around, requiring developer maintenance effort; worse, necessary code might be deleted, causing subtle or spectacular bugs.
