On the building contractor's side, the project's superintendent might be new to the field so they won't catch errors of subs or in the architect's drawings. Saw that plenty of times.
The subs... sometimes they think they know everything.
And they all think the architect knows nothing.
And that's why the whole Grenfell report "the architect is at the center of the failure" is funny in a dark-humor way.
